These Terms and Conditions govern your use of the Lifestream Dental & Wellness website and your relationship with our practice. By accessing our website or scheduling an appointment, you agree to the following terms. Please read them carefully.
All appointments at Lifestream Dental & Wellness are subject to availability. Appointment requests submitted through our online form are not confirmed until you receive direct confirmation from a member of our team by phone or email. Confirmed appointments are held for the patient named in the booking only.
Please arrive 10–15 minutes before your scheduled appointment time to complete any necessary paperwork. Patients who arrive more than 15 minutes late may need to be rescheduled to ensure that all patients receive adequate time and attention.
We reserve the right to decline or discontinue treatment at our discretion. We are committed to providing care in a safe, respectful environment and will not tolerate abuse or inappropriate behavior toward any member of our team.
We require a minimum of 48 hours' notice to cancel or reschedule an appointment. This allows us to offer your appointment time to another patient in need of care.
Cancellations made with less than 48 hours' notice, or appointments missed without any notice (no-shows), may be subject to a cancellation fee. Patients with a pattern of late cancellations or no-shows may be required to prepay for future appointments or may be discharged from the practice.

Lifestream Dental & Wellness accepts most major PPO dental insurance plans. We submit claims directly to your insurance provider on your behalf; however, the patient is ultimately responsible for any amounts not covered by insurance, including deductibles, co-pays, co-insurance, and non-covered services.
We will make every effort to verify your insurance benefits before your appointment and provide you with an estimate of your out-of-pocket costs. These estimates are based on information provided by your insurer and are not a guarantee of coverage. Final payment responsibility rests with the patient.
Payment in full (or applicable co-pays) is expected at the time of service unless a pre-authorized payment plan is in place. We accept cash, personal checks, and all major credit cards. Financing is available through CareCredit for qualified applicants.
Accounts with outstanding balances may be subject to finance charges and may be referred to a collections agency after a reasonable notice period.
Your privacy and the security of your protected health information (PHI) are of paramount importance to us. Our collection, use, and disclosure of your health information is governed by our Notice of Privacy Practices, which is provided to all patients at their first visit and available upon request at our office.
This website does not collect personally identifiable information except where voluntarily provided through our contact form. Information submitted through our website is used solely for the purpose of responding to your inquiry or booking request and is not shared with or sold to third parties.
