Lifestream Dental & Wellness provides general dentistry, cosmetic dentistry, dental implants, Invisalign®, teeth whitening, sedation dentistry, emergency dental care, preventive care, hormone replacement therapy (HRT/TRT), medical fat loss, and peptide therapy. Each service is subject to a separate treatment plan and patient agreement established at the time of consultation.
By submitting your information through any form on this website, you consent to being contacted by Lifestream Dental & Wellness via phone, SMS, and email for purposes including appointment confirmations, appointment reminders, treatment updates, and promotional offers.

Lifestream Dental & Wellness uses automated marketing and communication tools, including GoHighLevel, to manage appointments, send reminders, and improve the patient experience. All appointments are subject to availability and may be adjusted due to emergencies, provider conflicts, or other circumstances outside our control. Appointment requests submitted online are not confirmed until you receive direct confirmation from our team by phone or email. Please arrive 10–15 minutes early to complete any necessary paperwork.
We require a minimum of 48 hours' notice to cancel or reschedule an appointment. Cancellations with less notice, or missed appointments without notice, may be subject to a cancellation fee. To cancel or reschedule, please call us at (772) 206-0084 during business hours (Monday–Thursday, 9 AM–5 PM).
All estimates provided — whether verbal, written, or digital — are non-binding until confirmed during a formal consultation or in a signed written agreement. Final costs may vary based on your specific treatment needs, insurance coverage, and other clinical factors.
Payment in full (or applicable co-pays) is expected at the time of service unless a pre-authorized payment plan is in place. Deposits may be required before certain procedures begin. Payment schedules for larger treatment plans are discussed and agreed upon during the planning process. Late or missed payments may incur additional charges. We accept cash, major credit cards, and patient financing through CareCredit.
Patients are responsible for providing accurate and complete health information, arriving on time for scheduled appointments, following post-treatment aftercare recommendations, and maintaining open communication with our team. Failure to do so may affect treatment outcomes for which Lifestream Dental & Wellness cannot be held responsible.
Your personal and health information is handled in accordance with our Privacy Policy and all applicable laws, including HIPAA where applicable. Please review our Privacy Policy for full details on how we collect, use, and protect your information.
